摘要: 本文归纳孙轶秋教授治疗儿童紫癜性肾炎（孤立性血尿型）的经验:本病的病机总属本虚标实，初期以湿热、瘀血为主，后期以气阴不足为要；治疗以扶正祛邪，标本兼顾为原则，根据病机主张分期论治，治以清热利湿，活血化瘀，益气滋阴，凉血止血，固护营卫，预防外感；提倡慢病慢治；附验案1则以佐证。

    Abstract: This article introduces Professor Sun Yiqiu’s experience in treating Isolated Hematuria in Children with Purpura Nephritis. The pathogenesis of this disease always belongs to deficiency in origin and excess in superficiality, In the early stage, belongs to damp heat and blood stasis; In the later stage, belongs to the deficiency of Qi and Yin. The treatment should be based on the principle of strengthening body resistance and eliminating evil and considering both the specimen and the specimen. According to the pathogenesis, the treatment should be treated by stages; Clearing heat and promoting diuresis, Promoting blood circulation and removing blood stasis; Benefiting Qi and nourishing Yin, Cooling blood for hemostasis; Solid business health, Protecting the Yin-wei and guard against exogenous infection. Advocate slow treatment of chronic diseases. Giving one case to attached for evidence.

   紫癜性肾炎（HSPN）是儿科较为常见的继发性肾小球疾病[1]，是继发于紫癜引起的系统性血管炎所致的肾脏损害，临床表现以血尿、蛋白尿为主要表现。本病发病机制尚未明确，加之诊断治疗不够统一，因而在HSPN患儿中预后存在较大差异，研究表明HSPN诱发慢性肾衰竭在儿童病例发生率可达1.8%，累积成人慢性肾衰竭比率甚至高达10.41[2]。而在HSPN中尤以孤立性血尿型最为常见，西医治疗以糖皮质激素及免疫抑制剂治疗为主，疗效欠佳，循证指南建议孤立性血尿型HSPN仅对过敏性紫癜进行相应治疗，进行长期随访监测患儿病情变化[3]，目前对孤立性血尿仍无可靠的治疗手段。
